Support Services

Homeless Outreach.  Outreach services are extended on Maui and Kauai to connect homeless mentally ill persons with mental health services and other resources, including entitlements, to assist them in moving to transitional or permanent housing.

Case Management.  Active and intensive case management services are provided on Maui to individuals who need support and assistance in community recovery. The care team interacts weekly for case review/consultation; medication group and as needed for specific training issues. In addition, program staff interacts regularly and are part of a team consisting of the person served, a psychiatrist, family members and any involved mental health provider as indicated. Relief staff may be used to cover in case of staff absences. Staff members are on-call for after hour coverage.

Services include:
Treatment planning; recovery plan assessments and implementation; individual and group counseling; medication information; and self-care education. 

Care Plan:
The case management team creates a care plan with input from the consumer, and other significant to the consumer. The Case Management Program provides staff support, needs assessment, goal planning, and skill building, to help adults with mental illness move toward more independent living.  Additionally Case management services provides treatment and restorative interventions to assist consumers to gain access to necessary medical and rehabilitative services to reduce psychiatric and addiction symptoms and develop optimal community living skills

Program goals:

  • To realize of recovery and vocation/personal goals.
  • To lessen/eliminate debilitating symptoms of mental illness and minimize or prevent recurrent acute episodes of the illness through rehabilitation and progressive treatment interventions.
  • To ensure that consumers have the basic needs and skills for sustaining community living and enhancing the quality of life.
  • To improve/establish new linkages with a variety of community services and mobilize the involvement of the consumer’s support network.
  • To maintain consumer engagement in treatment.
  • To promote harm reduction, substance use reduction, abstinence, and recovery for SPMI consumers with co-occurring substance abuse/addiction by providing substance abuse services.

Hours of Operation:
Services can be provided to consumers in their home, work, or other community settings. Services are available twenty-four (24) hours a day, seven (7) days a week
